THIS FORM IS REQUIRED TO FILE A CLAIM FOR THE ANNUAL CLOTHING ALLOWANCE. BASED ON THE TYPE APPLIANCE INDICATED BY THE VETERAN ON THE APPLICATION AND THE DISABLITY FOR WHICH SERVICE-CONNECTION HAS BEEN ESTABLISHED A DETERMINATION IS MADE AS TO WHETHER OR NOT PAYMENT OF THE ANNUAL CLOTHING ALLOWANCE IS WARRANTED. 38 U.S.C. 362.
