THESE FORMS ARE CONSSOLIDATED BECAUSE OF THEIR CONNECTION TO THE ORGANIZATION OF A NATIONAL BANK. THIS CONSOLIDATED GROUP OF FORMS IS APPROVED UNTIL AUGUST, 1982, WITH THE UNDERSTANDING THAT A REQUEST FOR EXTENSION WILL ADDRESS TTHE FOLLOWING COMMENTS: 1. ARE THE PROVIDED "SAMPLE FORMS" THE ONLY GUIDELINES PROVIDED TO THE BANKS FOR THOSE FORMS? WHAT INFORMATION ON THE "SAMPLE FORMS" IS MANDATORY? WHAT IS VOLUNTARY? 3. CAN THESE FORMS BE COMBINED IN ANY WAY? Should the Department wish to obtain an extension of this approval it is to provide a detailed description of the actual use of each data element and data showing how many applications were rejected during a representative period of not less than 12 months and including calendar 1981 because of the data reported in response to each questio or combination of questions.
