THE HCFA 149OU IS A COMMON CLAIMS FORM USED BY APPROVED ORGANIZATIONS, SUCH AS THE RAILROAD RETIREMENT BOARD, WHICH PAYS FOR PHYSICIAN SERVIC FURNISHED TO THEIR MEMBERS. THEIR ORGANIZATIONS THEN REQUEST REIMBURSEMENT DIRECTLY FOR THE MEDICARE CARRIER.
