A question might be inserted as follows: "Has there been any change in trainee or trained worker monthly rate of pay since last statement was submitted? Yes or No. If yes, answer questions 5A and 5B. If no, skip to 7 or 8." Please provide OMB with copy of revised form when available. NOTE: In any revision or extension of the clearance, OMB will require special justification for monthly reporting (see proposed regulations replacing OMB Circular A-40).
