Abstract:
THIS FORM IS USED BY VETERANS TO REINSTATE THEIR GOVERNMENT LIFE INSURANCE. THE DATA COLLECTED IS USED BY THE VETERANS ADMINISTRATION TO DETERMINE ELIGIBILITY OF THE APPLICANT FOR THE PURPOSE OF REINSTATEMENT. THE COMPLETED APPLICATION IS REQUIRED BY LAW 38 C.F.R. 823.
