AM RADIO BROADCASTING, FM RADIO BROADCASTING, TELEVISION BROADCASTING THIS FORM IS NECESSARY WHEN APPLYING FOR AUTHORITY TO CONSTRUCT A NEW OR MAKE A CHANGE IN AN EXISTING AM, FM OR TV COMMERCIAL BROADCAST STATION.
