THE USMA FORM 5-518 IS THE FORM GIVEN TO AN EMPLOYER BY THE CANDIDATE IN ORDER TO RECEIVE CREDIT FOR THE WORK DONE OUTSIDE THE SCHOOL/VOCATIONAL ENVIRONMENT.
