THE CASE MANAGER COMPLETES PARTS A AND B AND SENDS THE FORM TO TRAININ FACILITIES. THESE FACILITIES COMPLETE PART C TO CERTIFY THAT THE VETER OR ELIGIBLE CHILD (CHAPTER 35) BEGAN THE PLANNED PROGRAM.
