APPLICANTS FOR MORTALITY MEDICAL CODER TRAINING AND FOR VITAL REGISTRATION METHODS TRAINING COMPLETE AN APPLICATION FORM FOR USE BY THE INSTRUCTOR IN SELECTING TRAINING APPLICANTS.
