ACCREDITATION, SERVICE ORGANIZATION REPRESENTATIVE'. NEEDED TO ENABLE THE VA TO DETERMINE ELIGIBILITY FOR ACCREDITATION AS REPRESENTATIVE OF A RECOGNIZED SERVICE ORGANIZATION.
