FAA Form 8500-8, Application for Airman Medical Certificate or Airman Medical and Student Pilot Certificate, is approved contingent upon its uniform and sensitive implementation by knowledgeable, designated Aviation Medical Examiners (AME). AME reference documents, agency communications, and formal AME training workshops should provide appropriate instruction and guidance on implementation of the revised form. The FAA should continue to emphasize the need for discretion in the examiner/applicant relationship consonant with the agency's aviation safety mission and, further, should ensure that its own employees as well as designated examiners are cognizant of, and sensitive to, the concerns of all applicants regarding disclosure of personal information to a public agency. Remarks added, effective 01/29/91: The clearance includes modifications to questions marked as approved changes in this docket. Other FAA medical forms discussed in the September 1990 justification are also approved without change.
