This ICR is approved on the condition that a burden box (see 5 CFR 1320.21) be added application/reapplication questionaire. EPA should attach a copy of the questionaire with the ICR package. This ICR is approved for 63,648 hours. The 2,527 hours associated wit the proposed rule should be added through an ICW after the rule is published in the Federal Register. If the rule substantively changes at the final stage, EPA must submit a new ICR.

Abstract:
THE SECTION 301(H) PROGRAM INVOLVES COLLECTION OF INFORMATION FROM A MUNICIPAL WASTEWATER TREATMENT FACILITY AND THE STATE IN WHICH THE PUBLICLY OWNED TREATMENT WORKS (POTW) ARE LOCATED. THE POTW SEEKING TO OBTAIN A 301(H) WAIVER PROVIDES APPLICATION, MONITORING, TOXIC CONTROL PROGRAM INFORMATION.
