IF NARRATIVE APPLICATION IS USED, HOURS PER RESPONDENT IS 2 HOURS BASE ON 89 APPLICATIONS. IF MODEL PLAN FORMAT IS USED, HOURS PER RESPONSE I 1 HOUR BASED ON 88 APPLICATIONS.
