Abstract:
THE FORM IS USED BY SCHOOL OFFICIALS AND EMPLOYERS TO CERTIFY ENROLLMENT INFORMATION TO THE DEPARTMENT OF VETERANS AFFAIRS (VA). VA USES THE INFORMATION TO DETERMINE THE CORRECT RATE OF PAYMENT AND PERIODS OF PAYMENT TO ELIGIBLE PERSONS.
