Abstract:
THE AGENT ORANGE REGISTRY WAS AN AGENCY INITIATED EFFORT. THE APPROVED CODE SHEET HAS BEEN IN USE SINCE 1982 AND HAS PROVED TO BE A VERY CONCISE FORM IN GATHERING DATA FOR COMPUTERIZATION INTO REGISTRY DATA. THE INFORMATION IS OBTAINED THROUGH A PERSONAL INTERVIEW PROCES DURING PHYSICAL EXAMINATION.
