THE FORMS WILL BE USED BY INDIVIDUALS TO APPLY FOR APPOINTMENT IN THE COMMISSIONED CORPS OF THE PUBLIC HEALTH SERVICE (PHS) AND TO OBTAIN REFERENCE AS PART OF THAT APPLICATION PROCESS.
