This information collection request is approved as revised by CMS in its 04/09/03 email memo. OMB notes that the expiration date has been changed to 12/2005 to allow for completion of the effort to revise the form. CMS must submit a revised package for OMB review prior to implementing any changes. OMB also notes that the total burden for the collection is being adjusted to account for CMS' most recent burden estimate. As indicated by previous terms of clearance for this package, the UB 92 must be HIPAA compliant.
