Abstract:
The Centers for Disease Control and Prevention proposes to conduct continual national evaluation of the agency's Consumer Response Services Center--CDC-INFO. CDC-INFO is a proactive, unified, and integrated approach to the delivery of public health information. CDC-INFO is designed to contribute to improving the health and safety of the public. Customers are defined as any individual or group seeking health or public health information from CDC. This includes the businesses, researchers, and others. Customer interactions occur through multiple channels, e.g., telephone calls, e-mails, and postal mail and may occur in Spanish or English. CDC is seeking OMB approval of the revised version of the "Evaluation of the Centers for Disease Control and Prevention's National Contact Center (CDC-INFO) Information Collection Request. The evaluation approach described in this study has been revised to accommodate budget cuts; many of the survey instruments in the previously approved clearance have been eliminated.

Short Statement:
Due to CDC-INFO program budget cuts, 10 evaluation instruments approved in OMB 0920-0753 will be removed and are not requested as part of this revised package. Addition of survey item responses to align the CDC-INFO Web Survey for E-mail users and CDC-INFO Interactive Voice Response Survey to the U.S. Census 2010 short form with questions on race and ethnicity.
