Short Statement: There are multiple updates and revisions to 29 of the currently approved forms based on user feedback, changes in surveillance definitions; and Centers of Medicare and Medicaid Services (CMS) reporting requirements. A total of six forms will be removed from the package as they will be no longer used within NHSH resulting in the decrease in burden since last submission.
