Description/Changes for Common Form Usage:
The revised VA Form 21 requires the same information as prior version of the form. It is important for both VA's Office of General Counsel and the veterans service organizations themselves to have the additional information to contact the individuals. So the new form requires the individual applicant’s home address and, by removing “Optional,” requires the individual applicant’s business email address and business phone number. OGC had previously assumed that the service organizations separately collected this information. But OGC’s interactions with the organizations indicates that, often, the organizations rely on the VA form to contact the individuals and that without additional information, neither VA nor the organizations would be able to contact the individuals. Other minor revisions to the form are the options to indicate additional specific military service branches rather than having those listed categorized under other.
