Title: Requirements for Long-Term Care Facilities: Notification of Facility Closure (CMS-3230-F) 
Abstract: This rule ensures that, in the case of a facility closure, any individual who is the administrator of the facility provides written notification of closure and the plan for the relocation of residents at least 60 days prior to the impending closure, or if the facility's participation in Medicare or Medicaid is terminated, not later than the date the HHS Secretary determines appropriate.
