Title: Revisions to Requirements for Discharge Planning for Hospitals, Critical Access Hospitals, and Home Health Agencies (CMS-3317-F) | |
Abstract:
This final rule revises the discharge planning requirements that Hospitals, including Long-Term Care Hospitals and Inpatient Rehabilitation Facilities, Home Health Agencies, and Critical Access Hospitals, must meet to participate in the Medicare and Medicaid programs. The rule also implements the discharge planning requirements of the Improving Medicare Post-Acute Care Transformation Act of 2014 (IMPACT). It also addresses one provision regarding patient rights in hospitals that was proposed on June 16, 2016 (81 FR 39447). |

A notice may publish in the Federal Register before the 3-year deadline extending the timeline by 1-year. We do not intend to delay publishing the final regulation if we are able to publish it sooner. |
