OMB is approving this information collection to reinstate it so that the agency may discontinue it. No later than six months from approval of this reinstatement, the agency will conduct a non-substantive change request to 2502-0169 to ensure that the burden associated with the FUP, which was previously captured in this information collection, is instead captured in 2502-0169. To achieve this, the agency should update the burden table in Question 12 of Supporting Statement A, make any necessary adjustments to the burden table in ROCIS, and upload any relevant instruments associated with the FUP.

Abstract:
Application for the Family Unification Program: makes Housing Choice Vouchers available to eligible families to promote family reunification. Youths 18 to 21 who left foster care at age 16 or older are also eligible to receive assistance under the program for a maximum of 18 months. Eligible applicants are Public Housing Agencies, who must work with a Public Child Welfare Agency to identify and assist FUP voucher recipients. Information collected will be used to evaluate applications and award grants through the HUD SuperNOFA process.
