Abstract:

This proposed rule would revise the discharge planning requirements that hospitals, inpatient rehabilitation facilities, long-term care hospitals, home health agencies, and  critical access hospitals must meet to participate in the Medicare and Medicaid programs.  The rule would also enact the discharge planning requirements of the Improving Medicare Post-Acute Care Transformation Act of 2014 (IMPACT).
