Title: ●Methods for Assuring Access to Care; Exceptions for High Managed Care Penetration and Rate Reduction Threshold (CMS-2406-P)

This rule proposes to provide an exception to the requirements for documenting Medicaid fee-for-service access to care for States with high managed care penetration rates.  The rule would also set a threshold for rate reductions and restructuring that are not subject to the analysis, monitoring and public processes required for larger reductions.
