FORM USED TO OBTAIN DISCLOSURE OF AND INFORMATION CONCERNING THE INVESTMENT DISCRETION EXERCISED BY CERTAIN INVESTMENT MANAGERS OVER ACCOUNTS IN EQUITY SECURITIES.
