OWNERS/OPERATORS MUST NOTIFY EPA OF CONSTRUCTION MODIFICATIONS, STARTUPS, SHUTDOWNS, MALFUNCTIONS, DATE AND RESULTS OF INITIAL PERFORMANCE TESTS.
