Abstract:
SECTION 221 OF THE SOCIAL SECURITY ACT PROVIDES FOR INFORMATION REQUIRED REGARDING THE DATES CERTAIN DISABILITIES BEGIN AND END FOR AN INDIVIDUAL WHO DOES NOT HAVE A PERMANENT IMPAIRMENT. THIS FORM IS USED TO OBTAIN STATUS OF THE BENEFICIARY'S CONTINUING DISABILITY CONDITION TO ASSURE EQUITABLE ADJUDICATION.
