Abstract:
THE APPLICATION CARD IS THE BASIC DOCUMENT USED IN THE USES LOCAL OFFICES FOR ALL NEW APPLICATIONS, INCLUDING PARTIAL APPLICATIONS, FOR INDIVIDUALS SEEKING ASSISTANCE IN FINDING EMPLOYMENT OR SEEKING EMPLOYABILITY DEVELOPMENT SERVICES. THE ES 516 IS THE AUTOMATED EQUIVALENT OF THE RECORD OF SERVICE SECTION OF THE ES-511, APPLICATION CARD.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
