Abstract:
THE APPLICATION FOR TRAINING IS THE OFFICIAL APPLICATION FORM USED FOR ALL TRAINING ACTIVITIES CONDUCTED BY THE CENTER FOR DISEASE CONTROL. THE INFORMATION REQUESTED ON THE FORM IS NECESSARY FOR THE ADMINISTRATIVE PROCESSING OF STUDENTS FOR COURSES AND ALSO FOR GENERATING MANAGEMENT STATISTIC REGARDING TRAINING.
