Abstract:
THIS APPLICATION FORM IS REQUIRED TO FILE A CLAIM FOR BURIAL BENEFITS INCLUDING TRANSPORTATION EXPENSES. THE INFORMATION GATHERED IS USED TO DETERMINE BASIC ELIGIBILITY AND WHETHER THE PERSON WHO PAID THE VETERANS BURIAL EXPENSES SHOULD BE PAID, OR IF EXPENSES ARE UNPAID, IF THE CREDITOR IS TO BE PAID. AUTHORITY IS 38 U.S.C. CHAPTER 23.
