ABSTRACT: THIS FORM IS USED TO GATHER COMPREHENSIVE INFORMATION REGARDING THE VETERAN'S CURRENT MARRIAGE AND ANY PRIOR MARRIAGES FOR THE VETERAN OR HIS/HER SPOUSE. ALSO, INFORMATION IS GATHERED FOR CHILDREN OF THE VETERAN AND ANY CHILDREN OF THE VETERAN'S SPOUSE. THIS INFORMATION IS USED TO DETERMINE ELIGIBILITY BASED ON DEPENDENCY WHERE APPLICABLE.
