Abstract:
SECTION 204(D) OF THE SOCIAL SECURITY ACT AND THE FEDERAL COAL MINE HEALTH AND SAFETY ACT PROVIDE THAT, IF AN INDIVIDUAL DIES BEFORE BENEFIT PAYMENTS HAVE BEEN COMPLETED, THE AMOUNT DUE WILL BE PAID TO PERSONS MEETING SPECIFIED QUALIFICATIONS. THIS FORM IS USED TO DETERMINE WHO IS HIGHEST ON THE PRIORITY LIST FOR PAYMENT.
