Abstract:
SECTION 1631(E) OF THE SOCIAL SECURITY ACT PROVIDES FOR INFORMATION REQUIRED TO ESTABLISH ELIGIBILITY FOR SUPPLEMENTAL SECURITY INCOME (SSI) BENEFITS. THIS FORM ELICITS INFORMATION REGARDING INCOME AND RESOURCES OF A SPOUSE OR PARENT TO DETERMINE ELIGIBILITY OF THE APPLICANT.
