SECTIONS 217 (A) AND (E) OF THE SOCIAL SECURITY ACT PROVIDE FOR INFORMATION REGARDING THE CREDITING OF MILITARY SERVICE TO THE WAGE EARNER'S EARNINGS RECORD. THIS FORM IS USED TO SOLICIT SUFFICIENT INFORMATION TO MAKE A DETERMINATION OF ELIGIBILTY.
