IRC SECTION 6011, AND CHAPTERS 21 AND 24 REQUIRE EMPLOYERS TO PREPARE AND FILE THIS FORM TO REPORT WAGES SUBJECT TO INCOME TAX WITHHOLDING AND FICA TAXES, AMOUNTS WITHHELD FOR INCOME TAX AND EMPLOYEES AND EMPLOYERS SHARE OF FICA TAXES.
