THIS FORM WAS DESIGNED TO PROVIDE THE MAXIMUM EFFICIENCY FOR FULL REPORTING OF DEBTOR INFORMATION FOR BOTH THE INSTITUTION AND THE FEDERAL GOVERNMENT.
