THIS FORM IS USED IN SUPPORT OF A CLAIM FOR DISABILITY PENSION. THE INFORMATION REGARDING FARMING ACTIVITIES IS NECESSARY TO DETERMINE THE AMOUNT OF INCOME TO BE CONSIDERED IN DETERMING TOTAL INCOME, AND TO DETERMINE WHETHER OR NOT THE CLAIMANT IS STILL CONSIDERED SUBSTANTIALLY EMPLOYABLE.
