Before using form DOD will (1) check whether serviceman must authorize bank to release information, (2) check applicability of Privacy Act to see if information will be in an existing or new systems of records, (3) prepare a cover letter to finalize institutions which explains program and indicates it is voluntary. Before resubmitting form DOD will check with Department of Education to see if they maintain any of this information. At time of resubmission, DOD will submit a status report on the problem.

Abstract:
THIS FORM IS CRITICAL TO IMPLEMENTING THE LOAN REPAYMENT PROGRAM, AN ENLISTMENT INCENTIVE PROGRAM AUTHORIZED IN SEC. 902 OF P.L. 96-342. SERVICE MEMBERS HAVE ENROLLED IN THE PROGRAM (RE: DD-2057-2 AND DD-2057-4) AS EARLY AS 1 OCT 80, BUT AS YET THERE IS NO FORM TO COLLEC DATA TO VERIFY THE SERVICEMEMBER'S ELIGIBILITY OR ENTITLEMENT. THIS FORM ACCOMPLISHES THIS. IT WILL BE DISTRIBUTED ASAP SINCE DOD IS LEGALLY LIABLE TO MAKE DISBURSEMENTS AS EARLY AS 1 OCT 81.
