PERSONS USING SPECIALLY DENATURED ALCOHOL IN THE MANUFACTURE OF A PRODUCT ARE REQUIRED TO SUBMIT THIS FORM. THE FORM REQUESTS INFORMATION REGARDING FORMULATION OF THE PRODUCT, COPIES OF LABELS TO BE USED, AND SUBMISSION OF A SAMPLE. THIS INFORMATION IS USED TO ENSURE AUTHORIZED USE OF THE ALCOHOL AND CONSUMER SAFETY PURPOSES.
