FORM 972 IS COMPLETED BY THE SHAREHOLDER AND FILED AS PART OF THE CORPORATION'S ANNUAL INCOME TAX RETURN. FORM 972 ACCOMPANIES FORM 973 WHICH IS EXECUTED BY THE CORPORATION.
