THE APPLICATION FORM IS USED TO APPLY FOR ADMISSION TO THE U.S. MERCHA MARINE ACADEMY. THE PRE-CANDIDATE QUESTIONNAIRE IS USED TO ESTABLISH INITITAL ELIGIBILITY AND TO PLACE THE CANDIDATE INTO THE DEPARTMENT OF DEFENSE MEDICAL SYSTEM.
