THIS FORM IS USED TO DETERMINE THAT A SHIPMENT OF WINE HAS BEEN LAWFULLY EXPORTED. IT DESCRIBES THE SHIPPER, THE PERSON TO WHOM SHIPPED, THE REASON FOR EXPORT OR USE OUTSIDE THE U.S., DETAILS OF THE SHIPMENT FOR TAX PURPOSES, AND CERTIFICATION BY A U.S. GOVERNMENT AGENT SHOWING EXPORTATION OR USE OUTSIDE THE U.S. SHIPMENT IS MADE IN BOND (WITHOUT COLLECTION OF TAX).
