DD FORM 1966. APPLICATION FOR ENLISTMENT -- ARMED FORCES OF THE UNITE STATES. DD FORM 1966 IS THE BASIC FORM USED BY ALL FOUR SERVICES FOR OBTAINING DATA USED IN DETERMINING ELIGIBILITY OF APPLICANTS FOR ENLISTMENT IN THE ARMED FORCES OF THE UNITED STATES, AND FOR ESTABLISHING RECORDS FOR THOSE ENLISTED.
