PROVIDERS OF SERVICES PARTICIPATING IN MEDICARE PROGRAM ARE REQUIRED TO SUBMIT INFORMATION TO ACHIEVE SETTLEMENT OF COSTS FOR HEALTH CARE SERVICES RENDERED TO MEDICARE BENEFICIARIES.
