THIS FORM ESTABLISHES THE CLAIMANT'S BASIS FOR A CREDIT REMISSION AND ALLOWANCE OF TAX OR A LOSS OF TAXABLE ARTICLES (ALCOHOL, SPIRITS, WINE, BEER, TOBACCO PRODUCTS, ETC.). IT REQUIRES THE CLAIMANT TO LIST THE REASONS WHY HE SHOULD GET THE CREDIT REMISSION, ETC. WITH THIS COMPLETED FORM AND SUPPORTING DATA REQUIRED, A DETERMINATION ON WHETHER TO GRANT THE CREDIT, ETC.
