THIS FORM IS NEEDED TO DETERMINE WHETHER THE CLAIMANT IS ENTITLED TO RECEIVE A REFUND FOR TOBACCO ARTICLES WITHDRAWN FOR EXPORT. THE FORM DESCRIBES THE TOBACCO ARTICLES FOR DETERMINING TAX TO BE REFUNDED AND EVIDENCE OF EXPORTATION BY THE CERTIFICATION OF A U.S. CUSTOMS AGENT.
