MSHA CERTIFIES MINERS AS QUALIFIED TO PERFORM CERTAIN DUTIES IN MINES. FORM 5000-1 IS USED BY COURSE INSTRUCTORS TO REPORT TO MSHA FOR CERTIDICATION THE NAMES OF THOSE MINERS WHO HAVE SATISFACTORILY COMPLETED ELECTRICAL AND NOISE TRAINING PROGRAMS.
