This information collection is approved subject to the following condition: Form MCS-90 must be modified as suggested on page 1 of the April 27, 1983 letter from the American Insurance Association to Gerald J. Davis of DOT. The MCS-90 must be revised to delete the last sentence of the first full paragraph ("The telephone number to call is: ______________"). The space for the insurance company telephone number should instead appear directly below the space for the name of the insurance company, near the upper left side of the form.

ALL MOTOR CARRIERS SUBJECT OT 49 U.S.C. 10927 WHO SATISFY THE REQUIREMENTS OF 49 CFR 387 WITH AN ENDORSEMENT OR BOND. THE ENDORSEMENT CONTAINS LANGUAGE NECESSARY TO OVERRIDE EXCLUSIONS OR CONDITIONS IN INSURANCE POLICY THEREBY EXTENDING PROTECTION TO THE PUBLIC INTENDED BY CONGRESS.
