Terms of Clearance:
This request for clearance is approved for 6 months to allow this reporting cycle to be completed within the prescribed time frame. Due to the short time frame and because all changes have been deletions of items which result in a decreased burden, publication in the Federal Register is waived. For this clearance the printing of the OMB expiration date is also waived. For an extension of this clearance, you must submit the request at least 45 days prior to the expiration date. You will not be granted another waiver from publication in the Federal Register and, unless you can sufficiently justify compelling reasons to omit it, you must publish the OMB expiration date on the form. Any request for extension must also include sufficient evidence that the burden is only one half hour per response or include a revision to that burden. The Paperwork Reduction Act notice must also be included in any future requests.

The following is a summary of the topics, regarding the proposed collection of information, that the certification covers:
